HIPAA-compliant privacy policy, notice of privacy practices, and No Surprises Act / Good Faith Estimate information for the practice.

Susan Quinn, LMFT, takes your privacy seriously. This Privacy Policy describes the information I collect through this website (susanquinn.net) and how it is used and protected.

What I collect. When you submit a contact or booking form, I collect the information you provide (typically your name, email, phone, and message). When you visit the site, standard server logs may record IP address, browser type, and pages visited. I do not use third-party tracking pixels or advertising trackers on this site.

How I use it. Information submitted through forms is used only to respond to your inquiry and to schedule services. Server logs are used only for security and basic site analytics. I do not sell, rent, or share your information with third parties for marketing purposes.

Clinical communications. Phone, email, and text are convenient but not fully secure communication channels. Please limit content of unencrypted communication to logistics — scheduling, basic questions. Sensitive clinical content is best discussed in session or through a secure client portal once you become a client.

You can request a copy of any personal information I hold about you, request corrections, or request deletion at any time by contacting me at info@susanquinn.net.

Notice of Privacy Practices (HIPAA)

As a licensed therapist, I am required to comply with the Health Insurance Portability and Accountability Act (HIPAA). My full Notice of Privacy Practices, which describes how protected health information (PHI) may be used and disclosed and how you can access this information, is provided to all clients at intake.

In summary: I use and disclose PHI only as needed for treatment, payment, and healthcare operations, and as required or permitted by law. Disclosures for any other purpose require your written authorisation. You have the right to access your records, request amendments, request an accounting of disclosures, and file a complaint if you believe your privacy rights have been violated.

Under the federal No Surprises Act, you have the right to receive a Good Faith Estimate of expected charges for medical and mental-health services. As an out-of-network provider, I will provide you with a written Good Faith Estimate of the expected cost of services upon scheduling, or upon request before scheduling.

If the actual cost of services is at least $400 more than the Good Faith Estimate, you have the right to dispute the bill within 120 days. For more information, visit cms.gov/nosurprises or call 1-800-985-3059.
